  • You have the right to an Individualized Program Plan (IPP) to identify your support needs.
  • You have the right to invite anyone you want to the meeting to help develop your IPP.
  • You have the right to tell others at the meeting what is important to you.
  • You have the right to refuse to consent to any part of your IPP.

Responsibilities 

  • You have the responsibility to participate in the development of your IPP.
  • You have the responsibility to tell your case manager who you want at your IPP meeting.
  • You have the responsibility to decide what is important to you.
  • You have the responsibility to work with the team to write a plan to which all team members can agree.
